Registering With Us


We are currently accepting new patients who live within our catchment area which is defined on the attached map. In order to fully register with the Practice all patients must:

  • Live within the catchment area, primarily NW2, NW3, NW6, NW11.
  • Provide their medical card duly signed, or
  • Complete an application to register form (GMS1 available at reception).
  • Provide two forms of identification.
  • Complete a Practice Health Questionnaire (available at reception).


  • You will be required to attend a registration and health screening appointment with the practice nurse, speak to our receptionist a couple of days after you have completed the  questionnaire and she will make an appointment for you. 


  • Please note that you will not be able to book a routine appointment until you have had a health screening appointment with a nurse. Health Screening includes measurement of height, weight and blood pressure, a discussion about diet, smoking, alcohol consumption and exercise. Women will be offered a breast examination and a cervical smear.


  • Any patient between the age of 16 and 74 who has not attended the surgery in the past three years may request a health check appointment. Patients aged 75 and over should be seen at least once a year by one of the clinical staff. Please contact the surgery to book an over 75 health check if you have not been seen in the past 12 months. 


  • It is important if you are registering a child that you provide us with details of their previousimmunisations, preferably the red book.



Each family member must fill out a separate form and please bring one official document and one proof of address on your first visit. Acceptable documentation is listed below.  


Acceptable Official Document

  • Your NHS medical card (if missing please contact your previous doctor for your NHS number)
  • Passport 
  • Birth Certificate 
  • Driving License

Acceptable forms of proof of address

  • A recent utility bill (within the past 3 months) showing your name and address 
  • A recent bank/building society statement (within the past 3 months) showing your name and address 
  • A tenancy agreement/mortgage documents showing your name and address
