Our Responsibilities to You

  • To treat you with respect and courtesy at all times.
  • To discuss with you the care and treatment we can provide.
  • To give you full information on the services we offer.
  • To give you the most appropriate care by suitably qualified staff.
  • To provide you with emergency care when necessary.
  • To refer you to a consultant acceptable to you when necessary.
  • To allow you access to your health records, in accordance with current legislation.
  • To hold all information in the strictest confidence. Your personal health information is confidential and no information will be given to a third party without prior consent, except in the case of medical emergency when relevant information may be required.
  • To give you a full and prompt reply to any complaint you may make about our service.

Your Responsibilities to Us

  • To treat all the staff at the surgery with respect and courtesy at all times. Violent or abusive behavior may results in your being asked to find another doctor
  • To ask for out of hours visit or emergency treatment only when absolutely necessary.
  • To keep your appointments and contact the Practice in advance if you are unable to attend.
  • To take responsibility for your own health and to follow advice from doctors and nurse.
  • To take medication according to the instructions, keep follow-up appointments, review repeat prescriptions with doctors or nurse, and attend for screening procedures.
  • To notify us when you change your address or telephone number.
  • The practice is registered with the Data Protection Agency and is bound by the rules governing the collection and storage of personal data. Your personal data will only be seen by professionals at the practice involved in providing your care. Occasionally anonymised health information is sent to the Primary Care Trust to support quality monitoring, public health analysis and post-payment verification. Under the Data Protection Act 1998, you have the right of access to your health records. If you wish to be given a copy of your records a small fee will be charged for this service to cover the cost to the practice. If you would like to apply for access to your records, please speak to your doctor or the practice manager.
